Tretinoin and moisturizer: which goes first?

Follow your prescriber’s instructions. Corneo schedules the rest. The American Academy of Dermatology (AAD) says a dermatologist may suggest moisturizer first to reduce irritation, but only with their help [3]. So tretinoin goes first unless your prescriber says otherwise.

What the labels say

The Retin-A label for the cream and gel gives the clearest order [1]:

  • Apply once a day, before bedtime [1].
  • Wash with a mild soap, then wait 20 to 30 minutes. The label says skin must be completely dry to minimize possible irritation [1].
  • Use about half an inch or less of the medication [1].
  • Apply a moisturizer, or a moisturizer with sunscreen, every morning after you wash [1].

The newer Retin-A Micro label, revised in 2025, is similar [2]. Cleanse with a mild, non-medicated cleanser and pat the skin dry [2]. Apply a thin layer once daily in the evening [2]. To manage irritation, it says to apply a topical moisturizer [2]. It does not say whether moisturizer goes before or after tretinoin [2].

When moisturizer goes first

The AAD lists three ways a dermatologist may reduce retinoid irritation [3]:

  • Apply a smaller amount of medication.
  • Use the retinoid less often.
  • Apply moisturizer first, especially if your skin needs a stronger retinoid.

The AAD adds that you should only make these changes with your dermatologist’s help [3].

Studies have used both orders. In one study, a ceramide moisturizer went on before tretinoin 0.05% gel each evening, and the authors reported the regimen was safe and effective [5]. In another, people used tretinoin 0.05% cream at night and a moisturizer with SPF 30 each morning on one side of the face [6]. Most had some irritation on both sides, about 83% to 86%, but it was mostly mild on the moisturizer side, and tolerability favored that side [6].

Not every source agrees on order. One retinol maker’s guide says to follow retinol with moisturizer [7]. That guide addresses retinol, not prescription tretinoin [7].

Whichever order your prescriber picks, keep it consistent, and talk to them before you switch.

Why moisturizer matters either way

The AAD says tretinoin and similar products tend to dry and irritate the skin, and a daily moisturizer can help skin tolerate them [4]. It suggests products labeled oil-free, non-comedogenic or “won’t clog pores” [4]. Applying moisturizer after washing helps trap water in the skin [4].

What to keep off tretinoin nights

The Retin-A label advises caution with several products [1]:

  • Medicated or abrasive soaps and cleansers.
  • Soaps and cosmetics with a strong drying effect.
  • Products high in alcohol, astringents, spices or lime.
  • Products with sulfur, resorcinol or salicylic acid.

The Retin-A Micro label adds that over-the-counter products with benzoyl peroxide, sulfur, resorcinol or salicylic acid may increase the risk of irritation [2].

Sun and weather matter too. The label says to minimize sun exposure, use sunscreen and protective clothing, and notes that weather extremes such as wind or cold may irritate the skin [1].
